As a new patient or a patient who has not been seen by us in a year or longer, you may see a Care Coordinator for your first appointment. In these cases, the Care Coordinator will take in your medical history, review your needs with you, and help schedule you for therapy, psychiatry or another service. Your next appointment will be an evaluation by a provider.
The purpose of an Initial Appointment is to evaluate your high-level behavioral and mental health needs and get you scheduled for your next appointment, which is an evaluation with a mental health professional who can begin building a treatment plan with you.
Some insurance plans may require you to have a referral, though most do not.
Once your appointment is scheduled, you will get a notice from us in the Patient Portal to complete your paperwork. If you’re a new patient, you’ll may meet with a Care Coordinator to go over appointment paperwork, have your initial appointment and then get scheduled with your provider from there. If you’re an established patient, you will still get a request from us to complete paperwork.
